New Jersey will issue an amended birth certificate upon receipt of "a certified copy of an order from a court of competent jurisdiction which indicates the name of the person has been changed" and "a medical certificate from the person's licensed physician which indicates the sex of the person has been changed by surgical procedure." Stat. Ann. §§ 26:8-40.12. To apply for an amended birth certificate the applicant should submit a written request to change the name and/or gender on birth certificate, a notarized letter from a physician on physician's letterhead indicating that surgical procedures are complete and including the doctor's license number, a copy of the individual's original birth certificate, a certified copy of the court ordered name change if applicable, and a check/money order for the applicable fees (payable to NJ State Treasurer) to: New Jersey Bureau of Vital Statistics and Registration, Attn: – Record Modification Unit, PO Box 370, Trenton, NJ 08625.
