Seriously though, his vitriol towards his parents pisses me off. My dad's dead, my mother left the country and I've hardly heard from her since. I'd love to be able to have a normal relationship with them again. Phil has that opportunity and not only actively shuns it, he shamelessly lies about his folks for attention. Apparently in Phil logic, asspats are thicker than blood. To me, it's easily one of the most utterly contemptible things about him. Nothing is sacred to him except his next fix of attention.

That seems like something the doctors/caseworkers would want to know about.
That suggests the gender dysphoria may not be inherent , but is a product of a deeper issue due to a traumatic event. Essentially, he is fleeing from a past event surgically
At the very least, Phil should get counseling to resolve that connection between gender identity and traumatic events.
I have trouble believing that, if seen, a MHP would sign off on surgery given that state-of-mind
