Their ego doesn't allow them to think the people who don't like them could possible feel anything else besides jealousy or hate, let alone have something besides laughing at tards on the internet in their lives. Life is a zero sum game that centers around cows in their minds. You're hateful because the cow is amazing and your life centers around hating the cow.
Personally, I only really hate certain people I have dealt with in real life. Lolcows are not worth "hate". Hating someone takes effort and is sometimes painful. Not wasting that on Tommy.

People like Tommy are desperate to avoid introspection, taking any kind of responsibility for themselves, acknowledging any kind wrongdoing, etc. So, they build walls of ego and delusion... and eventually become prisoners in those walls when they get so high that they can't see the outside world. Now, Tommy is reflecting on the outside what he is in the on the inside: a haggard, crazy old man, driven mad from isolation and the fear of what lurks outside the defenses he's built up.
The worst punishment for Tommy that I could think of is him, on his death bed, suddenly sitting bolt upright and realizing "It wasn't the rest of the world... it was me." And having his final moments consumed by decades of regret.
