Phil, define "bottom surgery", it makes it sound like you're getting ass implants
Maybe a doctor kiwi can correct me if I'm wrong but also doesn't taking hormones increase weight gain for awhile, without proper exercise and nutrition? If he some how gets hormones and his poor diet...Jesus christ.

That's the ethical thing to do, but I imagine the doctor would hesitate for a moment. Like, "What am I gonna do, not castrate this guy?"
"Bottom surgery" is a euphemism for the procedures for SRS. Orchiectomy is usually the first step and it's something a lot of MtF people do even years ahead of the rest of it since it means you can get off androgen blockers.
But yeah, I don't see any American doctor doing it. You can skip a lot of the gatekeeping if you fly to somewhere like Thailand or somewhere for it where there are actually quite a few surgeons with good reputations who'll do it, but they're expensive. Very expensive. Knowing Phil, he'll probably hop a bus down to Tijuana or something.

Yep. That's why most respectable trans surgeons won't even look at you without proof from a shrink that you're trans, letter from your doctor that you're ready and that you've been on hormones for X amount of time, and a clean bill of health. Of course, Phil could circumvent that by going out of the country for it, but I think that'll be too much effort for him.
Nope. The vocal cords change during puberty and SRS (or even HRT) can't reverse that. That's what voice therapy is for - basically training your voice to be feminine. Basically the thing that lets this happen:

That's the common phrasing for trans people who choose to get surgery to make their genitals look like the genitals of their preferred gender.
Idk how it works in California, in Oregon it depends on the surgeon. The wpath standards of care dictate that you have a relationship with two therapists for 3 months and get letters from those two therapist stating that you are mentally well enough for surgery. The surgeon that I'm seeing waives the 3 month quirement but still requires two letters. Some surgeons require you know the therapist for 6 month but only want one letter. It can very pretty widely.
A humane doctor would just put him down during surgery.
