An issue where images are added as attachments when they shouldn't and don't show in the editor -

The attachments don't show in the editing window and so can't be deleted. I've had this problem several times in the past, most recently it happened here:

It doesn't show attachments in the bbcode editor. Just delete the inlining tags. You don't even need that. Then delete duplicates.

Save and then refresh. For some reason whatever script updates the attachments and adds them to the inline editor doesn't always refresh the list. It used to do this all the time in Chrome, but they fixed it. It's probably some variant of the same thing.
