In an actual war with actual combat medics, that actually is a war crime and generally frees the other side to ignore such insignia and just indiscriminately kill anyone wearing it, as they have forfeited their protections under the Geneva Convention. That's why some asshole pariah nations/terrorist groups/Palestinians instead abuse the insignia of organizations like the Red Cross/Crescent because you really aren't allowed to attack them for the same reason and it's bad optics if you make a mistake and accidentally do.
(Unless you are Israel in which case you do not give a single solitary fuck.)
