The NHS is really weird about what it counts as cosmentic and non cosmetic surgery, They wont pay for purley cosmetic surgery so if they dont deam you sick enough to live a "normal" life without a surgery you want you cant get it on the NHS and they just have some healthcare that the wont cover.
For example they will pay for you to have weightloss surgery if your morbidly obease but wont pay for the nessasary skin removal that comes with morbidly obease people becoming a normal weight.
Some experemental surgerys they also wont cover.
IIRC Katie Price or someone similar did manage to get a boob job on the NHS but she would have had to spend alot of time throwing a fit about it to prove she was under enough mental destress to need one with most normal people cant afford to do
