He was fairly quiet when he was volunteering at the Long Haul, so the only thing I can think of is the row with 'holiday' was the last straw and he got kicked out - or at least told to take some time off.

The very idea of someone being an "allist" is absolute nonsense. If this were taken at face value it would mean that someone would have to be on the side of "isms" that are against them as well! I still don't get why Phil thinks we want to stop his surgery. I'm 99% sure that everyone here (if not all, then most) WANT him to get the surgery. Most people want him to get it so he realizes balls weren't what were making him unhappy...and then blame something else. Many people want him to get the surgery for the sole reason that he will be unable to have kids after.
Many of us do not consider Phil a latinx woman. Many of us consider Phil a white male. This surgery isn't going to change anything. He will still have his penis. In fact, he will still have his balls as this surgery is never going to happen because no doctor in their right mind is going to give Phil his recommendation letters.
(And even IF it did happen, no hospital is going to go through all the shit Phil wants them to... they'll say "yeah sure I'll tell X about that..." and nothing of value will be lost. Phil will be just as accessible in the hospital as he is now.)
