Anyway I called it when he first showed off the stinkditch - his poor diabetes care is probably interferring with the healing of what is alerady a horribly mangled open wound. My guess about the "clitoris" falling off is that the tissue went necrotic from poor blood circulation and possibly poor hygiene, and just fell off. He did not bleed to death like he (correctly) thinks he should have because it was all necrotic and not getting any bloodflow at all.
The only other option I can think of for the case of the disappearing clit would be if his body healed so well and so quickly that the "clit" got healed into the body and is now mostly a mass of scar tissue.
Given this is Johnathan Jessica Yaniv Simpson we are talking about, I'm leaning towards the first option.

Patients paying you to mutilate them and getting weekly updates on how their bodies rot more and more with time. This is like the wet dream of some horror movie scientist or butcher. The best part is that he literally admits it's his fault at the end so i guess that's a voice clip that could be exploited later in a courtroom.
