There's some ongoing quiet conversations in healthcare about basically what the fuck to say to get them to actually cover things they're supposed to cover.
Cancer risk? "Meh, we don't expect to be paying for you when that happens because we don't think more than a month ahead." Back pain? "LOL pain does not exist!!!1!" Back damage? See answer #1.
If your insurance offers 'alternative' medicine coverage, you might be offered for any of these the option of treatments which are 100% woo 0% effective...but cheaper than a reduction.
Dysphoria? "Awww you poor tranny let us get those off right now. We don't want to look transphobic, after all!"
