That face was him mugging for the camera. I always thought he was uncomfortable eating on stream, as opposed to the prerecorded stuff where he was Chef Phil.
I personally believe he would complain about "sensitive teeth" or something if he was having any mouth pain. He's even complained about temporary sporadic pain with his wisdom teeth.

Not since first discovering him in 2011 have I ever heard him mention visiting a dentist for any work or even a routine checkup. I imagine on some level his teeth and gums are fucked and it's just another facet of his lazy/unhealthy lifestyle that he's subsisted on greaseball fast food that goes down easy. Hell we know he likes the penne because it's "easy to eat" and who the fuck even says things like this in their 20's as though they have dentures and take issue with actually chewing food rather than just necking it like a fat italian toad.
