I would say any chance for a Bully 2 or a similar type of game taking place in a school or university.

The title, Bully, was enough to stir such an uproar that PAL regions wanted it changed to Canis Canem Edit. Even before its release, people hauled it as a "Columbine simulator" because of its open ended nature to roam a school. Supposedly, it would even enforce bullying towards schoolchildren.

Oh, and you could kiss a boy and do a panty raid. Imagine something like this in today's political climate with school shootings and the LGBT+ community.

The quest "Brygid the Stylist Returns" in Final Fantasy XI. In fact, I'm not really sure why it was considered acceptable back then, either. She wants you to kill some undead and bring back a certain "subligar" that they drop (there are thirteen different ones). Despite the name, they actually look like panties when your character wears them, hence back in the day people would get together groups for a "panty raid". When she gets it, she comments enthusiastically about its qualities, including the aroma (!!!). What makes this unacceptable, though, is the fact that Brygid is a little girl.

I wager the nineties were worse, in the sense that gaming mags killed Earthbound stone dead.

I'll never forget one Dreamcast ad, though. The official magazine had a two-page spread for Chu Chu Rocket, and one of the co-op players is a frowning, Uncle Remus type called "Peaches" who writes I'm nude in the chat.

I have less experience with 90s era magazines, but in looking at scans honestly it seems like they had a more neutral tone and doubled down on the "teenage boy" vibe in the 2000s, with PSM being an exception though, which is what those scans are from (albeit from 2002), PSM were putting cheesecake comic book illustrations of video game girls on the cover of their magazines since the 90s.

I said "shit" as in "stuff" because I love old video game magazines, however they weren't perfect, they were often overly critical of games, some of my favorite games of all time got bad reviews, gaming journalism has even at the best of times been something you needed to take with a grain of salt.

Heck, I remember a critic in OPM giving the original Drakengard a bad review solely because he found something in the game's storyline offensive, which anticipates the modern Kotaku style of gaming journalism, but that was in 2004.

Found a big stack of my old magazines not too long ago and it was fun to flip through. Forgot PSM would do a swimsuit issue which would never fly but would also be totally unnecessary today. Also to bring up OPM reviews that missed the mark they gave Jak 2 something like a 3.5 out of 5. Sometimes they get it right too though. Jak 2 is another infamous review that sticks out in my memory, they gave Gary Steinman, who was the resident JRPG geek, an action platformer to review and he really struggled with it, giving it a meh review simply because he found it too hard.

Jak 2 certainly had some tough moments but I never found the game hard enough to knock points off it's score for, dumb review.

Ghost Hunter is another game I was thinking of, OPM trashed it, but I found it interesting enough to actually buy the game and I loved it, your instincts were always better trusted than critics.

It's cool that PSM liked the game though, but I was never a regular reader of them unlike OPM, I would only get PSM on occasion, so I must have missed their review.
