It might just be the lighting but he has an even more pronounced dead-fish-stare in the more recent photo.

Due to all that odd placement of fat, his head really does look like a capsule pill. I also notice that those buttons on Phil's analog of "The Classic" are just as haphazardly placed as the tattoos.
While other issues of Phil's appearance are relatively easy to fix (via diet, exercise, and better hygiene), should Phil come to his senses about the tattoos, it's going to require a lot of expensive laser surgery to get them all out.
