It’s that she’s laying down. All the skin and face fat gets pulled back instead of down. You can see the effect for yourself if you lay down and try to take a decent selfie.
So he literally stood over her laying down, snapped a photo, and posted it saying “see!? She’s giving me grief!”

it's so weird how barb went from looking frail and malnourished in the last pic to looking morbidly obese (and dead) in this one.
