Sliced my chin open going down a slide head first when I was a kid. It was a stainless steel tube slide that curved around and I guess there was an edge somewhere I hit my face on. I had blood gushing everywhere and there were like strips of skin and baby fat dangling off. My mom took me to a plastic surgeon so I wouldn't be sewn up by some ER butcher and they put 21 micro stitches in. only left a tiny scar

I'm lucky enough to have gone through life so far without having any major injury. I have had a few minor injuries though:

When I was like five I didn't like Spaghetti for some reason (five year old me was a retard who only ate pizza) so instead of telling my dad that I didn't like it I just flushed it all down the toilet. One day the plate accidentally slipped out of my hand and smashed against my big toe on my right foot and I had a lot of explaining to do at the hospital. It left a ugly scar and to this day my toe still looks like it has downs syndrome.

A couple of years later, me and my brother were playing after school and because we had no sense of space because we were dumb kids, we ran into each other. This made us both somehow cut open the skin around our eyebrows. We both have a scar from it - thankfully my scar isn't really all that visable but my brother's is pretty noticeable.

Finally, at thirteen or fourteen I managed to trip over my own leg whilst playing hockey at school. I hurt my elbow and was sent to the hospital (so I basically got a free day off school since this happened during the first period). I only ever so slightly fractured my elbow and didn't need to wear a bandage or anything but I put my arm in a sling for like a week to milk sympathy from my teachers and I used my arm as an excuse to not do any sports for a couple of months even though it stopped hurting for the most part like a day or two later unless I was opening and closing my arm a lot (my PE teachers called me out on my bullshit but they didn't care at all since I deliberately made my self terrible at PE since I didn't really care about sports).

When I was 12 there was this old pontoon that had a rope on it that everyone would swing off of. I thought It looked fun so I swang on it forgot to let go and slammed my leg on its old ladder and I cut my leg deep enough you could see the tendons good thing it was the best outcome though as if I let go any earlier I would have launched and mutilated my body on the sharp rocks and or smashed my head against the metal bridge that connected the pontoon to the shore.

Once broke my ass.

Butt seriously, one time whhen I was little, I was dicking around with my friends, climbing up the roof of my friend's house using this big old tree. There was a small hop from the Tree tothe roof. It was hard to miss, yet somehow I managed it. Fell almost 2 stories and landed squarely on my ass.
Managed to fracture my Pelvis.
Had to get surgery, was in a wheel chair for ages and people would wait on me hand and foot.
All in all a fun experience.
7/10, though would rather not break ass again.

Haven't had any major accidents. Cracked my chin open as a kid, and I think that was the most painful. Not just the fucking around that caused it, but I swear the needle the urgent care used to put a local anesthetic in was the biggest damn thing I've ever seen. That hurt worse than the actual injury.

I had this utterly ridiculous incident where I fell on my shoulder, I had it x-rayed and even though I was in a lot of pain I never went to the doctor , since I hadn't broken bones and I was busy with my job anyway. The trauma wasn't particularly severe per se, but because of this lack of care it evolved in borsitis and tendinits, and then in adhesive capsulitis. My shoulder was completely paralyzed, I couldn't even lift my left arm enough to touch my forehead. I had to go through months of physio to regain mobility, and it has been incredibly painful and time-consuming. I'm still in pain every now and then, even after 3 years.

My worst injury got reported on the news, so I am not going ro talk about that one.
2nd worst was stepping a rusty nail through foot as a kid, I left about a mile long trail of blood on my way home:)

I've had it pretty good so far as far as physical injuries go. Never even got any bones broken, only slightly fractured at the surface. I was occasionally cut by the local gypsy kids, one even electrocuted me with a live wire jutting out of the wall in the dressing room of our ghetto ass elementary school, but none of that had left a mark. Maybe there are a few remnant scars you can spot if you're specifically looking for them (using magnifying glasses is advised), but that's about it. All things considered, I'd say I've gotten off cheap.
