I'm not sure, the BBFC used to certify games in the UK before we got the PEGI ratings, and it was a bit weird. Some things would be banned outright and get no certification (Manhunt 2 for example) and a lot of games that you'd think would be rated 18 actually got a 15. I remember getting the original C&C free in a bundle of games with our first Windows computer when I was a kid and that has a cutscene where Kane shoots Seth point blank in the head so probably that, but then it is the BBFC so I may be misremembering. If not then the original Half-Life, that was pretty gory for the time and I think that was an 18. It's strange how we absolutely did have the video game panic in the '90s and '00s that the US did, but the BBFC didn't crack down on it anywhere near as strictly as it did the "video nasties" of the '80s.

It was either Mortal Kombat 4 or Resident Evil 2 on the PS1. I remember Resident Evil 2 scarring the shit out of me so badly I ran out of the room the first time Leon died.

Probably because unlike movies, games are not legally required to be rated. Some stores would probably refuse them though.
Some games like carmaggedeon fucked up and willingly got rated by the BBFC and they got banned until they turned all the pedestrians into green glow in the dark CIA niggers.

For me, it was Resident Evil: Director's Cut on the PS1 back in 1997.

I was literally four years old at the time, but my parents were fairly permissive due to being that perfect mix of White trash hillbilly rednecks and Gen X liberal counterculture types (my Dad was a Metalhead/Rocker and my mom was a Goth who also had Metalhead tendencies)

Seriously, I used to play a lot of M rated games and watch R-rated movies even when I was a little kid. The old mantra I always heard "Watch it, but don't repeat it."

My childhood was interesting to say the least
My mom had that view on entertainment unless it was M/R rated. I had to hide the content of games like Psychonauts from my dad for a long time.

Funny, because he took my sister's Snoop Dogg CD and kept it for himself. Playing AC/DC for me when I was in the womb was probably his idea, too.

Wolf3D at a friend's house, shareware Duke3D downloaded over dialup (like 7mb but it took ages), and Turok as the first M-rated game I actually owned

The first M-rated game I ever actually bought on my own was Silent Hill on PS1, and fortunately the cashier didn't give a shit and just typed in whatever when the cash register asked if I was 17. Thank goodness the cover wasn't violent.

Mortal kombat 9 or Infamous. I think my parents intentionally went out of their way to make sure they weren't sheltering me. The only thing they objected to, I guess, was sexual stuff, but MK9 had some pretty skimpy outfits.

You know what? Turns out I was wrong because thinking about it further DOOM 64 would have been the first M rated game I played, but I only played it briefly.

MGS2 was still the first M rated game I completed.

Pretty sure Doom was M-rated. If not that then Mortal Kombat 1 or 2 (whichever was M-rated first).

I -think- Wolfenstein 3D was rated M? I don't know, I believe the version I played predated the ESRB, my first M title may've been Resident Evil 2 if that one doesn't count.

I believe it was Halo 2.

Compared to other games then and now, the Halo games seem relatively tame, so I'm not sure where the M rating came from.

I was a nintendo kid back in the day who wasn't interested in stuff like Eternal darkness or RE4 so I guess the closest thing was from when I learned how to pirate VNs on PC as a teenager and I played the original Fate/stay night with porn in it.

Dunno, ratings weren't really a thing over here while I was under 18, and parents trusted me to decide for myself what I can handle anyway.
I guess Doom was the first bloody game I ever played, as it was like the fourth game I ever played after Scorched Earth, Tunneler, and Crystal Caves.
