IIRC, I remembered playing the Demo version of Unreal Tournament 2003 on computers at CompUSA as my first M-rated game (I forgot if was rated T or M, but given how it has gore), back when that brand of stores was a thing.

Either GTA1 or 2 for the playstation. Predating the ratings I once played a Strip Poker game I found on a floppy disk where the age verification was just answering a few pop culture questions from the 60s and 70s and I guessed my way through it.

Halo 1 technically, but that was at a cousin's house and I certainly didn't play long enough to get how the game played. I think I got The Godfather, Gun and RDR with my first Xbox.

I defintely remember the first M rated game I purchased, which was Half-Life 2. This was 2006 so I needed my mom with me to do it. Its sort of hilarious to think about that now since only a few years later my little cousins who were about 12 and 14 would be playing GTA 5 with all of the deranged violence and titties bouncing around in that game. HL2 is downright fucking tame, I have no idea how that game got an M rating with its hilarious ragdoll physics and total lack of gore.

I would imagine the original Mortal Kombat in the 1990's. Regardless if it was in the arcades or at home, Mortal Kombat was a blast to play. Even the toned down SNES version didn't ruin the fun for me.

Resident Evil 2 but if we're going to be technical, it's probably Mortal Kombat.

Or if we're talking about the age of when the E Rating was K+A, Splatterhouse would probably be considered. Or even before that with Elvira on the Amiga.

It's never quite clear how you killed someone.

Still, it's an arcade port, and back then, that meant something. I wonder if the Genesis version suffered from having so few buttons. You had to go third-party in those days. For instance, I had the ProPad:

Resident Evil at a friend's house. We thought we were sneaky by playing it when his parents weren't home. Unfortunately, it was his older brother's copy, and that dude was a prick. He blabbed about it to his parents, who blabbed it to my parents. Luckily, my parents were cool and I didn't get into trouble, save for them saying they didn't want me to go against my friend's parent's wishes. Maybe if it were an R-Rated movie or porn, then I might have gotten into trouble.

Meanwhile, my friend wasn't allowed to play video games for a week. Good times.

I'm not 100% sure. I played Mortal Kombat for Sega Genesis, but that was before ESRB existed. I'm going to go on on a limb and say Mortal Kombat II for SNES. My friend up the road had it and I used to walk up there and play it all the time. Actually, he wasn't so much my friend as much as his parents had a pool and bought him all the newest games and used to take us to Dairy Queen.

Come to think of it, my first time 'playing' Silent Hill 1 was watching a friend play it. We take it for granted now, but at the time it was unlike anything you'd ever seen. The look of the Otherworld hospital was great, the way it mirrors Alessa's festering wounds, and the bloody catwalks peering at floors below.

I really like the emergency hammer. Makes you cringe when you cave in someone's head with it. I didn't expect Harry to swing the pointy end.

Leisure Suit Larry had one of these. It's still putting in work apparently.

Also I'm pretty sure I Have No Mouth and I Must Scream was released unrated because the developers were worried about catching an A rating from the ESRB. That game was deeply fucked up.
